All changes and information updates can be sent by email, fax or mail.

It is mandatory that all original enrollment forms for new employees and all original changes of beneficiary forms are sent to MATRIX.

All health claims should be sent directly to The Co-operators. If your dentist has the capability, dental forms can be sent electronically to The Co Operators, otherwise dental claims should be mailed to The Co-operators.

Plan Member Online Services provide plan members with a flexible, self-serve way to find answers to routine questions related to their health and dental plan. Through a secure login, (link to follow) they can access general information about their coverage and specific information about their claims. Plan members can use Online Services to find:

  • The benefits available under the group plan

  • Limits, coinsurance and deductibles

  • Claims history

  • Claim status
